If you are ready to begin services, please download the Intake Form below, fill it out, and send it back to us with the additional documents listed on the form. We will contact you within 3 business days. If you do not hear back from us, please make sure to check your junk folder.

Once we receive all the documents needed, we will contact your insurance and request for authorization to assess. This may take 10 business days depending on the funding source.  Once we receive the authorization to assess, we will contact you to schedule observations.

Your BCBA will finish the observations and write an assessment report with recommended goals, direct service hours, and treatment plan. This report will be turned in to your insurance. Once this is approved and we receive the authorization to start direct services, we will contact you and schedule sessions!
